Support Coordination

Above all Support Coordination will maintain the effectiveness of the supports the Participant is receiving and the efficiency of how the funds are spent. Their goal is to give support to the Participant so they are kept aware of everything that is happening. On the other hand, they can help with starting your plan, along with Early Childhood Partners and Local Area Coordinators (LAC).

Our Support Coordinators are multilingual, speaking English, Arabic and Farsi. This means we can easily convey your needs to Mainstream and Community Supports without the added cost of an Interpreter. We follow your NDIS Plan ensuring your funds don't run out before your Plan review.  This is kept in check whilst allowing you to access services that help you achieve your goals.

Our vision is to put you, the NDIS Participant at the forefront of where your NDIS Plan funds are spent. Level of Support

There are three levels of Support Coordination that can be included in your plan:
1. Support Connection
2. Support Coordination
3. Specialist Support Coordination

Depending on your Individual Goals, Plan Objectives and Aspirations you may receive funding for these supports in your plan.

1 - Support Connection

Firstly, Support Connection is a support designed to improve and strengthen your ability to connect with Funded, Informal and Community Supports. As a result these will help you utilise your NDIS Plan to its full potential, whilst putting you in a position where you are able to follow your goals and aspirations.

2 - Support Coordination

Secondly, Support Coordination is the next level in the Coordination of Supports. This is the standard type of Support Coordination and it essentially helps the NDIS Participant develop skills. These skills will certainly aid in Understanding, Implementing and Using your NDIS Plan. We allocate a Support Coordinator to you to help you to do the following: Maintain Relationships, Manage Service Delivery Tasks, Live more Autonomously and be involved in your Community.

3 - Specialist Support Coordination

Moreover, Specialist Support Coordination is the highest level of the Coordination of Supports. It is for NDIS Participants whose situations are more complex which require specialist support. In addition to the standard assistance of a Support Coordinator. A Support Coordinator will assist you in managing challenges in your Support Environment and paving the way for a consistent delivery of services and support.

How Support Coordination can assist you?

This support lifts a weight that is generally on the Participant, their friends and family and their support network, we take the hassle out of finding the right service provider for you and connecting you to them successfully. We monitor progress through these service providers to ensure they are the right fit for you.

It is beneficial to the Participant to understand what certain aspects of the NDIS Plan is and that is part of what we do, we want to help you understand how we are handling your funds, how they are going to benefit you and show you other alternative services that can ultimately help you reach your goals.

Meanwhile it tracks the progress of your NDIS Plan and will prepare you for your annual plan review, alongside that we are there to help solve any conflicts you may have and answer any questions you have regarding your plan and the NDIS.
