What is the NDIS?

The NDIS is the National Disability Insurance Scheme. It was legislated on the 1st of July 2013 and was fully rolled out by 2020.

It is a new way to help people aged 65 and under with a disability to get the support and care they require.

Types of NDIS Supports

The National Disability Insurance Scheme funds a range of Supports and Services which may aid in the following:

  • Education
  • Social Participation
  • Independence
  • Living Arrangements
  • Health & Wellbeing.

NDIS Eligibility

To be eligibile for the National Disability Insurance Scheme, a person must fit into the three criteria of:

  1. Age - Must be 65 and under.
  2. Residency - Must live in Australia and have an Australian Citizenship.
  3. Disability - Disability must be Chronic. (Long Lasting or Recurring)

What happens when I receive my NDIS Plan?

Once you receive your plan our Support Coordinators will connect you to all the services outlined in your NDIS Plan, this is carried out via our Support Workers and or Allied Health Professionals.

If you or someone you know fit into the three criteria, then rest assured our staff at OneCare Support Pty Ltd will help you gather and compile the documents that are required by the NDIA (National Disability Insurance Agency). One of those documents is the Access Request Form. We're there to answer any questions along the way and follow up with any issues you may have with your application.

What is an Access Request Form?

This form is one of the forms required to complete your application. An Access Request Form structures your entire application whilst informing you of any extra information you need to supply. It essentially takes simple data about the person and instructs you on whatever else you may need. An Access Request Form can either be mailed to you or downloaded from the button below.
